From clinical frustration to a platform serving thousands — one physician's journey to level the playing field.
The Problem
In 2023, Paul Jake was consulting for a network of family medicine practices across California. Like 230,000+ independent physicians across the US, they were drowning in administrative work. Two hours every night charting after the last patient left. Forty-plus hours a week on prior authorizations. A billing system that required three separate logins. And a $4,800/month EHR contract designed for hospital systems, not a 6-provider clinic.
“These doctors became physicians to help people, not to fight with insurance portals until midnight,” Paul recalls. “But every tool on the market was either built for enterprises with IT departments, or so stripped down it created more work than it saved.”
The Spark
At a Stanford health-tech hackathon, Paul met Arjun Patel, a machine learning engineer who had spent five years building AI systems at Google Health. They stayed up until 3 AM sketching what a “practice OS” could look like — a single platform that could handle documentation, billing, compliance, and patient engagement without requiring an IT team to set up.
They interviewed 50 more physicians over the next two weeks. The pattern was unmistakable: independent practices were being crushed by administrative overhead, and no one was building modern, affordable software for them. The tools existed for hospitals. The tools existed for tech startups. But for the solo internist in suburban Ohio? Nothing.
The Solution
MediFlow was born from that frustration. What started as a weekend prototype — an ambient AI scribe that could auto-generate SOAP notes from patient conversations — grew into a full Practice OS with six integrated modules. The first pilot clinic saved 15 hours per week within 30 days. The second clinic reduced claim denials by 34%.
Today, over 5,000 practices run on MediFlow. We've processed over $2 billion in claims, saved physicians an estimated 1.2 million hours of paperwork, and helped independent practices compete with health systems 100x their size.

The idea is born
Paul Jake and Arjun Patel meet at a Stanford health-tech hackathon. After interviewing 50+ independent physicians, they discover a massive gap: enterprise-grade practice tools that small clinics can actually afford and use.
First prototype shipped
Built the first Ambient AI Scribe module in 8 weeks. Deployed to 3 pilot clinics in San Francisco. Physicians reported saving 2+ hours per day on documentation within the first week.
Seed funding raised
Raised $3.2M seed round led by a16z Bio+Health and Floodgate, with participation from Y Combinator and angel investors including former Athenahealth executives.

Paul is a healthcare technologist and serial entrepreneur with over a decade of experience building SaaS products at the intersection of AI and clinical operations. Before founding MediFlow, he led product engineering at two health-tech startups, where he saw firsthand how independent practices struggle with fragmented software and rising administrative costs. He holds a degree in Computer Science from UC Berkeley and an MBA from Stanford GSB. His mission with MediFlow is to give every independent practice the operational leverage of a large health system — without the complexity or cost.
